Tivdak’s product information contains a requirement that changes planning for its use: an ophthalmologist’s examination before the first infusion and when clinically indicated. The information published by Spain’s AEMPS places part of the work outside the moment of administering the medicine. Preparing access requires organising that care pathway. For Genmab, whose 2025 annual report describes responsibility for Tivdak’s commercial operations in Europe, the economic consequence is specific. An authorisation does not by itself create the capacity to care for patients. Commercial planning has to recognise the resources the system needs to use the product in accordance with its documentation. ## Authorisation opens a decision rather than settling every one The EMA records European authorisation on 28 March 2025 and lists Genmab A/S as its holder. The indication is monotherapy for adults with recurrent or metastatic cervical cancer progressing during or after systemic therapy. That boundary matters: total new cases of the disease do not represent the population meeting the indication. Other decisions remain separate from authorisation. The European Commission retains national responsibility for pricing and reimbursement in its explanation of the health technology assessment framework. A European regulatory record cannot establish Spanish funding conditions or availability in a hospital. The distance between authorisation and availability is visible in other medicines without applying it to Tivdak. For a cohort of 56 oncology medicines authorised by the EMA in 2021–2024, IQVIA and EFPIA record 50 available in Italy, 41 in Spain and 38 in France as of 5 January 2026\. Availability includes limited arrangements and establishes neither actual use nor hospital capacity. A regulatory date is a verifiable fact. Funding needs its own record. Service preparation requires knowledge of people, resources and procedures. These are documented requirements, rather than a description of current practice in Spanish hospitals. Their economic meaning requires separating administration time from the full care pathway. Arranging an examination, communicating a result and providing a referral route require work. Counting only infusion time would leave out activities that allow treatment to be delivered under the specified conditions. Clinical professionals own that work. The manufacturer can supply current documentation and answer appropriate product questions; care decisions belong to professionals and the centre. Access strategy loses precision when an organisational need in healthcare becomes a commercial promise. The relevant information for a local plan is therefore which part of the pathway is ready and what resources completing the rest requires. Capacity can be discussed through specific activities: an initial examination, a check before administration and a referral where indicated. Capacity to deliver that pathway depends on coordinating those tasks before use expands. ## A budget must describe the service it will support Economic assessment needs to distinguish the medicine’s cost from the cost of providing associated care. They are different components and can fall under different budgets. Adding them without knowing the relevant population, anticipated use and local conditions would produce an impressive figure that is difficult to defend to the spending decision-maker. The alternative is a traceable plan: which requirement comes from authorised documentation, which input belongs to Spain’s context and which assumption remains unresolved. Separating those inputs identifies which decision can be made and which information is still missing. Genmab’s European opportunity becomes operational when authorisation, funding and care fit into the same pathway. The eye-check requirement shows where that work begins: before the first infusion, in a service prepared to provide it.
